Seed Knife Therapy (Brachytherapy)

Short Introduction Seed implantation with iodine-125 or palladium-103 seeds (brachytherapy) is a highly effective treatment for patients with cancer. Brachytherapy requires no surgical incision, offers patients a shorter recovery time, and has less chance of troubling side effects. For example, for prostate cancer, brachytherapy is an outpatient procedure and most patients go home the same day as their treatment. They can also return to their normal activities a few days after treatment. Seed implantation takes only 45 minutes to 1 hour. Seed implantation with iodine-125 seed gives a lower dose rate of radiation than palladium-103. Because iodine-125 works in your body longer than palladium-103, it is ideal for treating slow growing tumors such as most prostate cancers. The 125 iodine seeds-which have a half -life of 59 days-release a short-course of gamma ray. The seeds implanted into cancerous masses and nearby tissue radiate targeted cells and ultimately destroy cancer. This prevents unnecessarily exposing the whole body to radiation. (See Figures 1-9, below) According to images offered by CT or MRI, and using computational optimization techniques, a three-dimensional conformal treatment plan is made for making the correct dose calculation that is necessary for killing cancer cells. Then, the number and location of seeds implanted is calculated. Seed implantation is often performed percutaneously (through the skin) under guidance of ultrasound or CT, as well as direct-visual guidance during operation. Before an implantation, an ultrasound volume study is done to see the size and location of the cancerous mass as well as the surrounding organs. During the pre-planning stage, a predetermination is made of how many seeds a patient will need and exactly where they should be placed; these details are based on the size and shape of the cancerous mass. At the beginning of the implantation, the patient will be given local anesthesia. To guide the precise placement of the seeds, an ultrasound probe is placed on the cancerous mass to obtain an image of the mass. Usually 20 to 100 seeds are placed inside needles that are inserted into the masses.  The ultrasound probe ensures that the needles are guided with maximum accuracy based on the plan. Some men with prostate cancer may experience discomfort while urinating, or the need to urinate more frequently. These symptoms usually begin 1 to 2 weeks after implantation and gradually decrease over time as the seeds lose their strength. Drinking lots of fluids and avoiding caffeine and alcohol may help relieve these symptoms. A small percentage of men, particularly those who have had previous prostate surgery, may experience incontinence. Impotence may also occur in some men, particularly those over the age of 70. However, the rates of impotence and incontinence are lower with iodine-125 seeds than with other treatments. The needle electrode used most frequently is a 14-gauge, 15- to 25-cm long insulated cannula containing 10 individual hook-shaped electrode arms or tines. Upon deplopment, the array of tines extends out to a diameter of either 2.0, 3.0, or 3.5 cm. Using ultrasound or CT to guide placement, the needle electrode is advanced to the area of the tumor to be treated. Once the tines have been extended or deployed into the tissue, the needle electrode is attached to a RF generator and treatment is performed. Tumors less than 3 cm in greatest diameter can be ablated with the placement of a needle electrode with an array diameter of 3.5 cm when the electrode is positioned in the center of the tumor. Tumor larger than 3 cm requires more than one deployment of needle electrode. Typically, the array is placed first at the interface of one side between the tumor and nondiseased parenchyma, and then the array is reposited and re-deployed on another side at 2.0- to 2.5 cm intervals within the tissue. To completely destroy cancerous tussue, the needle electrode is used produce a thermal lesion that incorporates not only the tumor, but also nondiseased tissue in a zone 1 cm wide surrounding the tumor. Transarterial Chemoembolization in India at FUDA hospital

3-3 Combination of Cryosurgery with Transarterial Chemoembolization for Liver CancerABSTRACTTransarterial chemoembolization (TACE), by itself, is not associated with improved survival, however, can shrink the mass of liver cancer.Combination of TACE and cryosurgery is expected to yield better therapeutic efficacy.It is showed that sequential TACE-cryosurgery might improve outcome for the patients with liver cancer with the lower local recurrence rate at the ablation area and lower incidences of hepatic bleeding complication. INTRODUCTIONCryosurgery has been used for two decades for the treatment of many benign, malignant, and metastatic cancer [1-3]. More specifically, hepatocellular carcinoma (HCC) has been successfully treated either with cryosurgery alone or in combination with resection [4-6].Transarterial chemoembolization (TACE), by itself, is not associated with improved survival when compared with that of an untreated control group[7], however, it has been proved to shrink the mass of liver cancer. It is shown that sequential TACE-resection might improve outcome for the patients with large HCC [8]. Therefore, combination of TACE and cryosurgery is expected to yield better therapeutic efficacy.INDICATIONThe combination of cryosurgery with TACE is mainly adaptable for larger liver cancer, including primary and metastatic.TECHNIQUETACETACE was performed in an interventional radiology suite after cross-sectional images were reviewed[9]. A 6 French vascular sheath was placed into the femoral artery, and a 0.035 inch diameter Mickaelson catheter was advanced into the celiac and superior mesenteric arteries. Contrast was injected into the arteries during rapid-sequence radiographic imaging. Arterial branches supplying thetumor were then located. The venous phase was carefully examined for patency of the portal veins. The vessels were analyzed for the presence of anatomic variation. A 0.018 inch diameter Tracker catheter was advanced through the Mickaelson catheter to the arterialbranches that supply the tumor. Doxorubicin (50 mg) and Mitomycin (10 mg) were mixed with Lipiodol (4-15 ml), and the mixture was injected into the arterial branches until hemostasis was achieved. If the tumor had no shrinking at 2 weeks after the procedure, the second TACE was performed.CryosurgeryCryosurgery is performed with open ,laparoscopic and percutaneous approaches. Percutaneous cryosurgery is performed under guidance of real-time untrasound.CLINICAL DATAClavien[9] evaluated in a prospective study the safety and efficacy of this combination in cirrhotic patients with unresectable hepatocellular carcinoma (HCC). Fifteen patients were included in this study. All but one patient underwent one or several sessions of TACE before cryosurgery. Cryoablation was successfully performed in each patient. The patient who did not undergo preoperative TACE required reoperation for hemorrhage. Another patient with Child class B cirrhosis died postoperatively of hepatic and multiorgan failure. At a mean follow-up of 2.5 years, three patients had recurrence of disease, and 13 of 15 patients were alive with the longest survival time being 5 years. The actuarial survival rate at 5 years was 79%.Qian[10]reported thirty-four patients with histologically or clinically confirmed primary or metastatic hepatic carcinomas which were treated with TACE. One week to 1 month later, they were treated percutaneously under ultrasound guidance using cryosurgical system. All patients were followed up to determine serum tumor marker, CT scans, MRI images or ultrasound images. This therapy was performed in 34 patients including 32 patients with Child A, 2 patients with Child B and no patient with Child C. There were 28 patients with primary liver cancer and 6 patients with metastatic liver cancer. During the follow-up period of 3 to 15 months, 41.1% patients were recognized clinically cured because the serum tumor markers became normal, or CT scans and MRI images revealed that the lesion became completely necrotic. Forty-four percent of patients were recognized effectively treated.We[11] reported their experience. From March 2001 to December 2006, a total of 980 patients with HCC received intraoperative or percutaneous cryosurgery in Fuda Cancer Hospital Guangzhou, China, among whom, 660 received TACE before cryosurgery. A retrospective study is to invest the results of 310 patients who were treated with sequential use of TACE-percutaneous cryosurgery, in comparison with 110 patients who received percutaneous cryosurgery alone. Results are as followings.SurvivalDuring a mean follow-up of 36±17 months (range from 24 to 60 months), 129 patients (30.7%) showed no evidence of disease, 98 (23.3%) were alive with recurrent disease, 170 (40.5%) died of disease recurrence or spread, and 23 (5.5%) died of other diseases (infection of respiratory system in 5 cases, heart disease in 12 cases, renal failure in 3 cases, and unknown etiology in 3 cases). The overall 1-,2-,3-,4-,and 5-year survival rates are 72%,57%, 47%, 39% and 31%, respectively. The survival rates between sequential group and cryo-alone group were statistically significant different at 4 and 5 years after treatment(p=0.04) (Table 3-3-1)(Figure 3-3-1). Table 3-3-1. Cancer Microvessel Intervention (CMI)

When patients are treated with general chemotherapy, chemo medicine will naturally spread throughout the body. Unfortunately, the drug distribution is not limited to only cancerous areas, but healthy areas as well. For this reason, general chemo has strong side effects, and is not particularly successful as a therapy. Therefore, we promote transarterial chemo-injection instead of systemic chemotherapy. In this procedure, we insert a catheter and give chemo medicine through the artery that supplies blood to the cancerous area. The advantage is a big increase in drug density inside cancer within a short period of time. The disadvantage is that the high drug density inside cancer is difficult to maintain, because the chemo medicine will eventually spread to the rest of the body quickly via the bloodstream. However, in this procedure, the side effects are less than the effects associated with systemic chemotherapy. At Fuda, we are researching whether there is an effective way to keep the chemo medicine staying at the cancer site as long as possible, instead of spreading to the whole body, which may cause the many side effects. Cancer Microvessel Intervention (CMI) combines different chemo medicines into a fine grain particle. By using superselective catheterization, this fine grain particle will be distributed inside the artery which supplies the target cancer. The fine grain particle of chemo medicine will stay inside tumor tissue and maintain a high density for a long period of time. CMI is based on the following theory: there are a lot of gaps among the cancer capillary endothelial cells, and tight junctions among the normal capillary endothelial cells. The fine grain chemo particle will go inside the interstitial space of cancer tissue, which causes an increase in osmotic stress, selectively constricts the cancer micro-capillary, and then blocks the blood flow. Therefore, the fine grain particle will stay inside the tumor for a relatively long time. Meanwhile, because of its small size, there will not be embolism (blockage) in the normal capillary. General chemo by vein infusion If we give 150mg Oxaliplatin by vein infusion, there is only 0.125mg Oxaliplatin inside the tumor; the other 149.875mg will flow to other organs of body.
